  • Undergraduate Registration and Add/Drop/Withdraw Form
    You can use this form to add, drop, audit, or withdraw from courses. This form is submitted to the Office of the Registrar, once completed.
  • Undergraduate Course Approval Form
    If a student is missing a prerequisite, has a time conflict, or cannot normally register for a course for some reason, this is the form a student will use to get permission to do so. This form is also used for any Independent Study, Thesis, Special Topics, and Internship courses. Please see our FAQ for more information.
  • Graduate Registration Form -  The graduate registration process has changed. Please review the information on the Registration Guide page before submitting the form. This form is submitted to the Office of the Registrar, once completed.
  • Graduate Special Topics Request Form - This is for all directed readings, directed study or independent study course requests. The course outline and the graduate registration form must be attached.
  • Declaration of Major
    Students can use this form to declare their major for their degree. You can change your major with this form, as well. You will need to declare a major before the end of the semester in which you will complete 30 credit hours. Complete the form and return it to the Office of the Registrar. For Honours declaration, please contact advising@unbc.ca
  • Declaration of Minor
    Students can use this form to declare a minor. Declaring a minor is optional: if this is something that interests you, please speak with your Student Advisor.
  • Declaration of Certificate or Diploma
    If a student is planning on completing a certificate or diploma, they will need to complete this form and submit it to the Office of the Registrar.
  • Confirmation of Enrolment - Domestic
    If your financial institution, workplace, agency, sponsor, or some other place needs proof that a student is attending UNBC, they may ask for verification or confirmation of enrolment. Complete this form, pay the fee at the Cashier’s Office, and then submit the form to the Office of the Registrar. A request typically takes 5-7 business days to be processed, and can be picked up in person or delivered by fax, post, or e-mail. Please make sure you’re registered in the semesters that you request confirmation for.
  • Confirmation of Enrolment - International
    If you are an international student and need a way to confirm to your country or agency that you are attending UNBC, they may ask for verification of Confirmation of Enrolment. Complete this form, pay the fee at the Cashier’s Office, and then submit the form to the Office of the Registrar. A request typically takes 7-10 business days to be processed, and can be picked up in person or delivered by fax, post, or e-mail. Please make sure you’re registered in the semesters that you request confirmation for.
  • Application for Graduation - Undergraduate
    If this is your first time applying to graduate, please apply online through our myUNBC Student Account (and see our FAQ for more information). If you are an undergraduate student who has applied to graduate previously with the same curriculum, or a Bachelor of Social Work (BSW) student obtaining a Specialization, please use this form to apply to graduate (convocate) for the Convocation ceremony in May. Make sure the fee is paid at the Cashier’s Office, and then submit the completed form to the Office of the Registrar.
  • Application for Graduation - Graduate
    If this is your first time applying to graduate, please apply online through our myUNBC Student Account (and see our FAQ for more information). If you’re a graduate student who has already applied to graduate with the same curriculum, please use this form to again apply to graduate (convocate) for the Convocation ceremony in May. Make sure the fee is paid at the Cashier’s Office, and then submit the completed form to the Office of the Registrar.
  • Confirmation of Graduation
    If a student needs proof of their graduation (for a post-graduate work-permit, for International Invitation to Graduation, or even as proof of degree requirement completion before convocation), they can submit this completed form to the Office of the Registrar. These letters take 5 to 7 business days to process.
  • Change of Name Request
    If a student’s legal name changes since beginning their studies, they will need to use this form to update their student records. A name change request must be accompanied by an original or certified copy of supporting documentation, such as a birth certificate, marriage certificate, name change document, etc. This is done in person at the Office of the Registrar. Note: If you are also or have also been a UNBC employee, you will need to contact the Human Resources department, instead.
  • Parchment Replacement Request
    If your parchment was lost or destroyed, you will need to complete this form in order to get a replacement. As there should only be one copy of your parchment in existence at a time, this process is more in-depth, so please see our FAQ.
  • Certified True Copy Request
    If you need a copy of one your official documents (for example, if an institution needed a copy of your parchment), you would complete this form. This only provides a COPY of the form, and is marked as such. Please see our FAQ for more information.
  • Undergraduate Deferral Form
    If you are offered admission and would like to move your application to a future semester within the same academic year, you would complete this form and pay the deferral fee. For undergraduate, non-competitive programs only.
  • Letter of Permission
    A Letter of Permission (LOP) is a request to take a course at another Institution and have it transfer back to be used towards your UNBC degree.
  • Transcript Request
    An official transcript is a student’s complete academic record at the University of Northern British Columbia. Transcripts requests can be completed online or completed via a paper form.  
  • Authorization to Act on Behalf of
    Please fill out this form if you wish for someone to act on your behalf and/or have permission to ask questions about your account. Please ensure that the form is submitted via your UNBC email address. Authorizations are valid for up to one year, or until revoked, whichever comes sooner.  
  • Request For Late Withdrawal
    Compassionate Withdrawals are defined as unusual circumstances beyond the student's control which are unpreventable and unpredictable, which makes it impossible for a student to complete coursework, and affects academic performance. If you have explored all of your options with your instructor, advisor, etc., you may submit this form with an outline of your specific situation. These are considered on a case by case basis. You may obtain a form from the Office of the Registrar in Prince George or by emailing studentinfo@unbc.ca
