Northern Health is implementing a process of whole system primary healthcare (PHC) transformation with staff, physicians, and communities to improve population health and outcomes for patients, improve provider and patient experience, and contain costs (the triple aim). Municipal leaders are key to the development of community-based partnerships. With their involvement, along with the physicians in their communities, health services and structures have a greater potential to be re-aligned to better serve the needs of the population. This presentation of findings from the Partnering for Change study (2011-2016) examines how the engagement of municipal leaders is contributing to PHC transformation in northern British Columbia.
